Payment Policy

Thank you for choosing our practice. Please review our payment policy below:

  1. Payment Required Before Appointment
    Payment for all services is required in full prior to your scheduled appointment. Appointments will not be confirmed until payment is received.

  2. Accepted Payment Methods
    We accept payments by credit card, debit card, ACH bank transfer, checks, and eligible FSA/HSA cards.

  3. Insurance Billing
    If we are billing your insurance directly, you are responsible for any co-pays, co-insurance, deductibles, or non-covered services. Co-pays and known patient responsibility amounts are due prior to the appointment. Proof of insurance must be provided in advance.

  4. Self-Pay / Out-of-Pocket Rates
    For patients paying out-of-pocket, payment is required in full prior to the appointment unless a pre-approved payment plan is in place.

  5. Payment Plans
    Payment plans are available for eligible services with prior approval. Payment plans require a valid credit card, debit card, or ACH bank account on file for automatic recurring payments.

  6. Outstanding Balances
    Any outstanding balance must be paid in full prior to scheduling additional appointments unless a payment plan has been arranged.

  7. Returned Payment / NSF Fees
    A $30 fee will be charged for any returned ACH payments or checks due to insufficient funds or bank rejection.

  8. Missed Appointments / Late Cancellations
    Cancellations must be made at least 24 hours in advance to avoid a fee. Missed appointments or cancellations with less than 24 hours’ notice will be charged a $75 fee. This fee must be paid before scheduling future appointments.

  9. Delinquent Accounts
    Accounts unpaid after 60 days may be referred to a collections agency, and future appointments may be suspended until the account is brought current.

  10. Refunds
    Refunds for overpayments will be issued to the original payment method after account reconciliation.
